Hi, thanks for taking the time to make an incident report. Please note that we are not here to mediate between the general public or stakeholders and any particular consultor.
The purpose of this form is to report a breach to our code by representatives or legal entities such as organisations, charitable trusts or companies. You should think carefully about starting this process but we promise to treat you fairly and your claim seriously.
Once we’ve got your details, we will:-
- Acknowledge them;
- Start pre-investigation: this is basically a conversation between the founding Directors of ACEP to determine if there are any merits and sufficient evidence to start a case;
- Seek early resolve : A conversation with the member and the claimant about the claim if [2] dictates;
- Start a full investigation: If the breach is serious and not resolved at [3];
- Strike: If the complaint is upheld at [4] (Two strikes to escalate);
- Hold a hearing. If the member is at risk of losing their membership due to [5].
